Hello ,

The problem is mostly because kernel extension can't be loaded due to system prolicy.
If you do not deny the kernel extension during installation, you could go to "Security & Privacy" preference panel, there is prompt button to ask load the kernel extension of iSCSI initiator, press the Allow button, then you will be able to use.

User can also run the following command to troubleshot:
sudo kextutil -v 6 /Library/Extensions/KScsiPrt.kext
After run the above command, the kext will be loaded or appear in the "Security & Privacy" preference panel to ask user to "Allow".

I would also add that:

1. The published uninstallation instructions are incorrect and do not work for V.3

2. Even with no connected iscsi targets, I was getting bad audio choppiness every 30 ~ 60 seconds after installing KernSafe Initiator X V.3. After I manually uninstalled the KEXT and rebooted, these issues stopped. I can't say for sure it was caused by KernSafe but it was unacceptable.
